NOTICE: In the event that a person other than the student's parent, legal guardian, or emergency contact fills out and submits this form, District reserves the right to take any legal action for fraudulent submission and/or take disciplinary action against any District student(s) involved. We reserve the right to call home and verify the absence as needed.
This form cannot be used for reporting a student arriving late or leaving early. Please call your school's main office instead.
Absences may be deemed excused/unexcused as noted in the District's Absenteeism and Truancy Policy.
Only absences deemed "valid" as detailed in Section 26-2a of the Illinois School Code are designated as excused.
